Integrated care is a priority for Together We’re Better - Staffordshire and Stoke-on-Trent’s Sustainability and Transformation Partnership - and is a key benefit to the formation of Midlands Partnership NHS Foundation Trust (MPFT).

MPFT is moving at pace to realign its services and support integrated care at locality level, across Staffordshire and Stoke-on-Trent. Our services have developed a population profiling framework, which supports professionals to identify the most appropriate care and support for different elements of our population, and can be tailored for an individual’s need. Further work is now taking place to advance the framework and ensure it is clinically supported and co-designed with clinicians and professionals from across health and social care services.

To develop MPFT’s core offer of integrated care staff have used the framework to align the Trust’s community physical health, mental health and social care services to all 23 localities outlined within Staffordshire and Stoke-on-Trent’s Sustainability and Transformation Plan. We will continue to work with partners across the system to develop this at locality level.
