What is the Health Passport?

The Health Passport is a secure online tool designed to help parents and carers share essential information about their child with clinicians and professionals at MPFT. It allows users to record details such as the child’s preferences, needs, and any critical information that may support their care. Once completed, the Health Passport can be exported as a PDF and printed for use during appointments.

Why is it needed?

Children and young people often interact with multiple professionals across different services. The Health Passport ensures that everyone involved in their care has access to consistent, up-to-date information. It helps reduce repetition, improves communication, and supports personalised care by giving professionals a clearer understanding of the child’s needs and circumstances.

Parents and carers can update the Health Passport at any time, making it a flexible and responsive tool that reflects changes in the child’s health, preferences, or support requirements.

How to access it

The Health Passport is available online and can be accessed via smartphone, tablet, laptop or PC. Users will need to log in using their name, NHS number, and date of birth.
