The East Staffordshire Community Therapy Team is a team of physiotherapists, occupational therapists and rehabilitation practitioners. We provide home visits for adults who are house bound within East Staffordshire.
Who can we help?
Any person aged 18+ who has a GP in East Staffordshire, including those in care/residential settings, can be referred to our service.
We can see a variety of conditions ranging from people who are mobile to those who are bed bound.
Please note, to meet our criteria you must be Housebound AND/OR have a new health condition or illness that is impacting your mobility or function.
How can we help?
We provide comprehensive assessments in the home to establish current problems and aims for treatment.
We ensure high quality of care by ensuring we work towards shared goals based on problems identified in our assessments.
What to expect from your first visit?
Your initial visit could include assessments of:
- Strength
- Movement
- Balance
- Mobility aids
- Your ability to complete daily tasks such as, toileting, washing, dressing, preparing food, self-care
- Equipment
- Moving and handling techniques
- Hoist or transfer aids
- Provision of equipment from external equipment providers.
Following assessment, and together with you, we will set attainable goals, working towards these goals by developing a personalised rehabilitation programme.
When can we help you?
Our service is available Monday – Friday.
We aim to provide you with a convenient time for your appointment, however, please note our staff work flexibly and may not always be available.
Our phone lines are managed by our fantastic admin team Monday to Friday who will pass any messages to our therapists. Our contact details are 0300 323 0930.
We take referrals into our service from GPs, hospital staff, care companies and other community teams.
